Developing agentic AI tool to facilitate self-help scheduling of Express Heart Check at Elderly Centres across 18 Districts
Project Description
The Express Heart Check (https://expressheartcheck.org) program was founded as a not-for-profit, community-based, preventive medicine outreach program that provides heart and frailty (cardiogeriatric) assessments for older adults aged 60 years who are attendees of Elderly Centres across 18 Districts in Hong Kong. Attendees of Elderly Centres usually reside in underserved communities and are considered underprivileged, with high rates of financial difficulties, noncommunicable chronic diseases (NCDs) and frailty. Since 2021, the 3-part Express Heart Check program including i) public seminar on cardiovascular disease and heart failure, ii) Q&A session, and iii) cardiogeriatric assessments to over 75 Elderly Centres (or over 97 visits/revisits). To date, no fewer than 600 older adults have received cardiogeriatric assessments, and approximately 40 referrals were made to expedite access to specialists.
Over the years, it has become evident that arranging visits with Elderly Centres can be complex, and presents a challenge to increasing service capacity. With the advent of agentic artificial intelligence (AI) technology and its popularized use, we intend to leverage this technology to reduce barriers and improve not-for-profit services to the Elderly Centres through easier arrangements and improved access and trackability. This project will lead to the development of a web-based agentic AI tool and planner to facilitate Elderly Centres to self-help with scheduling of available times, and provide the Express Heart Check team with a global view of visit times, dates, venues, venue details, geographical coordinates, transportation and parking suggestions, and other logistical information to streamline efficiency.
